Government agencies and nonprofits worked with the mother of four girls found dead in their home last year, but the family didn't get the team effort needed, according to the D.C. Inspector General.
In January 2008, U.S. Marshals found Banita Jacks at home with the bodies of her four daughters, whom she is accused of killing.
A review of the case catalogues each interaction Jacks had with the District from 2005. It finds that the school system's response to the girls' absences was inadequate and that the Child and Family Services Agency failed across a wide spectrum.
The review also says it appears that D.C. police officers did not see the family as claimed in 2007 after a charter school social worker reported that Jacks was holding her children hostage.
